Submit Approved Participants

WFO — Wellness for Organizations

Welcome, coordinator. This page is where you submit the list of approved participant emails for your organization. Once submitted, the people on this list can place orders using your organization's discount code on jdmotion.com.

How it works

  1. Download Template B from the link J&D Motion sent with your discount code.
  2. Open Template B in Excel or Google Sheets. Fill in one row per approved participant.
  3. Read the Instructions tab in the workbook before submitting.
  4. Export the Participant Emails sheet as a CSV file (File → Save As → CSV).
  5. Choose the CSV file below and click Submit.
  6. You will see a confirmation page after submission. Duplicate alerts (if any) are emailed to J&D Motion automatically.
Important: Your organization name in Template B must match exactly what J&D Motion has on file. Even a small difference (extra space, different capitalization) will cause the upload to skip your batch. If you are unsure of the exact name, contact customersupport@jdmotion.com before submitting.

Upload your participant list

Required columns:

org_name, participant_email, first_name, last_name, submission_date, action

action column accepts either append (add to existing list) or replace (overwrite the list). All non-blank rows must use the same value. Leave blank to default to append.
